given you're posting on here, I'm guessing your below 30, so 5 years prolly feels like a lot, especially if you started or had just started transition at the start of the relationship
but if you're bending into a perfect fit for him, and he doesn't want you, you're turning yourself into a reminder of him when you drift apart anyway
make yourself an individual, and stop being a parasite to a host that wants you gone
you will feel infinitly better being yourself, and when you can find someone again who fits with YOU rather than having to mold to each other, you'll be a lot healthier mentally
