I had breast reduction surgery. The nipples were kept attached to some kind of like flap of tissue inside, while a few pounds of flesh were removed.
Prior to surgery my nipples felt like nothing. My nipples are more sensitive now than they were before.
I don't enjoy the sensitivity desu. But surgery isn't a nipple killer.

You can talk to surgeon etc. If quite flat chested keyhole surgery could be an option? There are things they can do for you to have the best outcome.
